As a cardholder at the Combined Community Library, I agree to:
- Be responsible for all items checked out on my card.
- Return materials promptly as required by state law.
- Pay all fees that accrue on my card.
- Notify the library of any changes in my registration information or loss of my card.
A picture identification and proof of residence are required.
If this card is for a minor, a parent or legal guardian must sign. If parents wish their minor
children to use their card, please list those names on the reverse of this application.
This card must be renewed every 5 years. Replacement fee for lost cards is $2.00.
